Crash Vol 409 Ethiopian au large du Liban (737-800)

Un avion d'Ethiopian Airlines s'est abîmé en mer Méditerranée dans la nuit de dimanche à lundi 25 janvier, peu après son décollage de l'aéroport de Beyrouth.

Selon les documents de vol, l'avion comptait 90 personnes (82 passagers et huit membres d'équipage), se trouvent à bord du vol 409 d’Ethiopian Airlines. Une chaîne de télévision libanaise locale avait précédemment fait état de 59 personnes à bord.
Le vol 409 d’Ethiopian Airlines a décollé à 02h10 (00h10 Temps universel) de l’aéroport de Beyrouth à destination d'Addis-Abeba. L'appareil, un Boeing 737-800, a disparu des écrans radar environ cinq minutes après son départ. Selon les premiers témoignages, « une boule de feu » a été aperçue au moment du crash au large du village de Na'ameh, situé à quelques kilomèttres au sud de Beyrouth.
La plupart des passagers sont de nationalité libanaise ou éthiopienne. Trois passagers ont la double nationalité, libanaise d'une part, et britannique, canadienne et russe d'autre part.
Dès l'annonce du crash, des sauveteurs se sont précipités sur les lieux du crash pour tenter de retrouver des survivants. Plusieurs hauts responsables libanais et des proches de passagers se sont rendus à l'aéroport international Rafic Hariri.

Lebanese air traffic control lost contract with the crashed Ethiopian Airlines Boeing 737-800 at 9,000ft following its departure from Beirut today.
The director of Lebanon's air navigation department, Khaled Chamieh, tells Air Transport Intelligence that the aircraft departed runway 21 on a service to Addis Ababa.
Weather data shows the presence of cumulonimbus clouds and thunderstorm activity, but Chamieh says that the jet climbed to 9,000ft before contact was lost.
"The aircraft was called several times but it did not respond," he says, adding that the investigation is underway and no further information is available.
Ethiopian Airlines has ended confusion over the identity of the aircraft, confirming it was an eight-year old model - serial number 29935, registered ET-ANB - leased from CIT.

The aircraft underwent maintenance on 25 December at the airline's facilities, says the carrier, and was "declared safe and fit to fly".
Ethiopian adds that the pilot had over 20 years' experience on various types across the carrier's network.
"A team of experts from Ethiopian has arrived in Beirut and has reported that the Lebanese Government and its army, together with the United Nations security forces in Beirut, are aggressively engaged in the search efforts for survivors at the accident scene," says the carrier.
Twenty-five victims have been recovered. Ninety passengers and crew were on board the flight, ET409.

Ethiopian 737 failed to maintain correct heading before crash

Preliminary information about the final few minutes of yesterday's ill-fated Ethiopian Airlines flight out of Beirut indicates that the aircraft was struggling to comply with air traffic control instructions to change heading.

The Boeing 737-800, bound for Addis Ababa as flight ET409, departed towards the south from Beirut's runway 21 in darkness, but initial information suggests that the jet first turned right to head north, over the Mediterranean Sea, and was cleared to climb to a minimum crossing altitude to transit through Syrian airspace to the east.

An air traffic control source familiar with the event tells Air Transport Intelligence that an exchange with Beirut centre discussed navigating towards the VOR beacon identified as CAK, which lies on the Lebanese coast north of Beirut.

But he says that air traffic control needed to ensure separation between ET409 and two aircraft which were on approach to Beirut's runway 16 at the time.

In order to maintain separation, he says, ET409 was instructed to turn left onto a heading of 270° but the crew apparently failed to comply.

"Instead it turned left, further," says the source, adding that - as the heading wound down to 140° - controllers became alarmed that the aircraft was turning back towards land and high terrain.

Controllers tried ordering the aircraft to turn right, warning the crew of the mountainous area, but the source says that radar contact was lost with the jet shortly afterwards. It came down off the Lebanese coast south of Beirut.

Meteorological data confirms the presence of thunderstorm activity but there is no immediate evidence that the conditions directly led to the loss of the aircraft.

Investigators have yet to retrieve either the cockpit-voice or flight-data recorder. The recovery effort has been hampered by poor weather.

But Ethiopian Airlines says that a specialised vessel is being brought in to assist, adding: "This, we believe, will facilitate an earlier recovery of the [flight recorders]. It will also help to find and lift the main fuselage of the aircraft. So far, part of the wreckage and some life rafts have been recovered from the area."

Ninety passengers and crew were on board the jet but rescue teams have not found any survivors.

With the loss of the Ethiopian Airlines Boeing 737-800 offshore from Beirut on 25 January, the phenomenon of fundamentally serviceable aircraft - and all their passengers - being lost over the sea at night is becoming frightening.
I have put this issue under the spotlight before.
Here's a list of the main airline losses in this category since 2000:

In some of them the cause has been established officially. Gulf Air, Armavia, Flash and Adam Air were all caused by a combination of total or partial pilot disorientation followed by a failure to control an aircraft that could have been controlled.
Yemenia is known to have hit the sea having stalled at quite low level during an attempted circling approach at night, so that could be put in the loss of control/loss of situational awareness category.
Air France was known to have suffered some specific technical anomalies, but in the absence of new information indicating the the aircraft was physically uncontrollable, the crew should have been able to maintain control but failed to do so.
And now Ethiopian Airlines. It sounds distressingly like several of the others, particularly the Flash Airlines event. In the latter, the captain was carrying out a slow turn over the sea at night, and from what he said to the copilot, he clearly had the "leans". The aircraft went into a spiral descent and crashed. The copilot could see what was happening but left intervention too late.
We have learned from Beirut air traffic control that the aircraft was on a northerly heading soon after take-off from runway 21. We believe it was heading to a coastal way point north of Beirut, possibly with the intent of crossing Lebanon eastward, bound for Syrian airspace. There it would, presumably, have turned south toward its destination, Addis Ababa.
But the flight, still in its early northward climb, was told to turn left onto 270deg to avoid traffic inbound to land on 16. The aircraft's left turn continued through 270deg around to 140deg, despite warnings from ATC. The crew did not respond, and radar contact was lost.
There were thunderstorms in the area, and this may turn out to be a contributory factor in what happened. But right now it looks terribly like another case of pilot disorientation over the sea at night.
This is an undeniable phenomenon now, but no-one is recognising it as such. If they were, determining more appropriate pilot training would be a priority internationally. Maybe the aviation insurance industry should start lobbying, but it's a pity they should need to.

Search teams locate recorders from Ethiopian 737 that crashed

Search teams have located the flight data recorder and cockpit voice recorder that was on board the Ethiopian Airlines Boeing 737-800 that crashed into the Mediterranean Sea.
The passenger aircraft crashed in the sea on 25 January, soon after take-off from Beirut airport, and reports in Lebanon - quoting the Lebanese military - say radio signals have pin-pointed the recorders' location.
The reports say the recorders are 10 kilometres from the airport and at a depth of 1,300m (4,300ft).
International search teams, including a US naval vessel, have been searching for survivors and wreckage. There were 90 people on the aircraft. No survivors have been found.

"L'avion a explosé en vol et sa carlingue, ainsi que les (corps) des personnes à bord se sont dispersés en mer, en différents lieux", a indiqué à la presse Mohammad Jawad Khalifé, qui expliquait la raison pour laquelle certains corps avaient été retrouvés démembrés. "Les corps repêchés en premier étaient intacts mais par la suite, on a commencé à remonter des membres de corps ou des corps défigurés", a-t-il ajouté à l'hôpital Rafic Hariri de Beyrouth.

C'est la première fois qu'un responsable libanais affirme publiquement que le Boeing 737-800 a "explosé", quelques minutes après son décollage en pleine tempête. Un responsable du ministère de la Défense avait affirmé le jour de l'accident que le Boeing s'était désintégré en quatre morceaux avant de s'abîmer. A la suite de l'accident, les autorités libanaises avaient immédiatement écarté l'hypothèse d'un "acte de sabotage".

L'une des deux boîtes noires repêchée avant-hier a été envoyée en France pour analyse, a indiqué aujourd'hui une porte-parole d'Ethiopian Airlines. Cette analyse doit permettre d'expliquer la raison pour laquelle l'appareil a dévié de la trajectoire indiquée par la tour de contrôle. Vingt-trois corps ont été retrouvés depuis l'accident.

BEYROUTH — La seconde boîte noire de l'avion d'Ethiopian Airlines qui s'est abîmé au large du Liban le 25 janvier avec 90 personnes à bord a été récupérée mercredi, alors que des responsables mettaient en garde contre des conclusions hâtives concernant les causes de l'accident.
"La marine libanaise a remonté la deuxième boîte noire" enregistrant les conversations dans le cockpit, a affirmé à l'AFP le ministre des Transports, Ghazi Aridi, trois jours après la récupération de celle contenant les données liées au vol du Boeing 737-800 qui s'est écrasé quelques minutes après son décollage de l'aéroport de Beyrouth à destination d'Addis Abeba.
"Les enquêteurs ont découvert qu'une partie essentielle de la boîte noire manquait et les plongeurs sont en train de la rechercher", a toutefois précisé ultérieurement M. Aridi.
La première boîte a été envoyée en début de semaine en France pour l'analyse qui doit permettre d'expliquer la raison pour laquelle l'appareil a dévié de la trajectoire indiquée par la tour de contrôle.
Le Bureau d'enquêtes et d'analyses (BEA) "confirme" que la boîte noire contenant les paramètres de vol (dite "FDR") a été envoyée et "lue", a déclaré à l'AFP à Paris une porte-parole du BEA.
Elle a néanmoins ajouté qu'il appartenait aux autorités libanaises de communiquer sur le déroulement de l'enquête.
La compagnie Ethiopian Airlines, pour sa part, a indiqué dans un communiqué "n'écarter aucune possibilité, y compris celle d'un sabotage", tout en rappelant que "l'enquête en (était) encore à ses débuts" et qu'il était "trop tôt pour conclure sur les causes de l'accident".
Le ministre de l'Information Tarek Mitri a lui démenti que le gouvernement libanais ait reçu un rapport préliminaire de France faisant état d'une erreur de pilotage, en réponse à des informations de presse.
"Nous n'avons pas encore reçu un rapport officiel sur le contenu de la (première) boîte noire, et tout ce qui est présenté comme des informations préliminaires sur ce sujet est imprécis", a dit M. Mitri à des journalistes.
Différents experts interrogés par l'AFP ont indiqué que le temps orageux pourrait ne pas être la seule cause de la catastrophe, l'avion ayant pu avoir un problème de moteur ou d'hydraulique. L'appareil a également pu être déstabilisé en entrant dans un cumulonimbus.
Des responsables libanais ont tenu des déclarations contradictoires, certains affirmant que l'avion avait explosé "en vol", tandis que d'autres ont affirmé que l'appareil s'était disloqué après avoir touché la surface de l'eau.
Vingt-trois cadavres ainsi que des membres ont été jusqu'à présent repêchés. Parmi eux, ceux de Marla Sanchez Pietton, épouse de l'ambassadeur de France au Liban, et du pilote ont été identifiés au cours des dernières heures.
